THRILL THE WORLD - PHOENIX VALLEY ZOMBIE DANCER REGISTRATION
This year we dance October 25th at 3:00 PM sharp! Please arrive by 1:00 PM to practice and officially sign into the dance floor.

Please register in advance and make it easier for all of our un-lives!

1. Please read the Waiver.
2. Fill out the form, make sure you check the box for Adult or Under 18. 
3. By selecting YES on the Responsibility Waiver form, you indicate that you have read the form and Waiver and agree (all liability stuff for all our partners, Thrill The World - Phoenix Valley and the volunteers).
4. We promise your contact information will not be distributed.

Registration fee: $5 donation per individual and $10 donation per dancing family to Phoenix Children’s Hospital. You are always welcome to donate more!! 100% of your donation is going to Phoenix Children’s Hospital!!!
